Forms & Documents

Forms A and B (fillable and savable PDF)

These medical forms are required for all Scouts and any adults participating in troop activities.  Forms A and B are needed for all troop events.

Form C

This form is only required for activities lasting longer than 72 hours, such as summer camp or other long-term programs.

The above forms expire on the last day of the month, one year after the date signed, so please be sure to renew them annually.  We’ll help you remember!



Activity Consent Form (permission slip)

This form provides parental permission for Scouts to participate in troop activities, trips, and outings.  It is typically used for events requiring additional approval, transportation details, or special arrangements, and is required for activities involving air travel.  Only needed if leadership requests it be completed.

Advancement & Merit Badges

Troop 1966 encourages Scouts to advance at their own pace while exploring new skills, interests, and opportunities. These resources can help Scouts and their families track progress, learn about merit badges, and plan their Scouting journey.  Unless otherwise noted, links are to official Scouting America sites and documents are current as of June 2026.

Tracking Advancement

Merit
Badges

Merit badges allow Scouts to explore hobbies, careers, and skills while working with counselors and troop leaders.

Scouts interested in pursuing Eagle can find helpful planning resources/requirements here.

Some Scouts stay for adventure.  Some for leadership.  Some for Eagle.
